Accessing CDAP with your MY TT CARD



The New MY TT Card is here and available right away for those currently on the CDAP Programme.

Persons on the Ministry of Health's Chronic Disease Assistance Programme (CDAP) now have an even easier way of accessing free medication with the MY TT CARD.  The MY TT CARD is a "Smart Card" that will allow you to access Government Services.  CDAP is the first government service to be offered via the MY TT CARD.


It's Cutting Edge!

MY TT CARD is a "National Smart Card" and will contain all your information protected by a secure method.


It's Easy to Use!

Instead of using CDAP Prescription Forms, your TT Card will come ready to use.  Just present it to the pharmacist.  It is safe and automatic!


It's Complete!
It is the new way to access Government Services starting with CDAP.  More Government services will be accessible in the future.



How do I get one?
To get a MY TT Card, you'll first need to have an electronic birth certificate.  The card is based on the new birth certificate's electronic identification.
 

If you DO NOT HAVE Your New Trinidad and Tobago Birth Certificate, follow these steps:
  • You need to apply for a new birth certificate through TT Post.  Go to any TT Post outlet and ask for a Trinidad and Tobago Birth Certificate Form.  Fill it out and post it.
  • Your new birth certificate will be ready to be mailed to you within two weeks if there are no queries or problems with your application.
  • Go to your pharmacy and show them your birth certificate and your current CDAP Prescription Forms.
  • You will get a MY TT CARD Application Form to fill out.
  • Submit it to the pharmacy.
  • Pick up your MY TT CARD in two (2) weeks (circumstances may vary).
  • Your MY TT Card comes ready to use immediately.

 

If you ALREADY HAVE Your New Trinidad and Tobago Birth Certificate, follow these steps:
  • Go to your pharmacy and show them your new computerized birth certificate and current CDAP Prescription Forms.
  • You will get a MY TT CARD Application Form to fill out.
  • Submit it to the pharmacy.
  • Pick up your MY TT CARD in two (2) weeks (circumstances may vary).
  • Your MY TT Card comes ready to use immediately.
